To request records, please download and complete the Medical Records Consent Form (PDF), sign it, and email it to MedicalRecords@AizerHealth.org. For the fastest service, we encourage patients to use our patient portal to submit requests and communicate securely with our staff. Please allow 7–10 business days for processing.
